The Form on the Fridge That Could Save Your Life
Johnson County’s MED-ACT team has a tool that makes that conversation a little easier and more practical. It’s called the File of Life form. You fill out your critical health information — medications, preferred hospital, your doctor’s name — slip it into a bright red sleeve, and put it on your refrigerator.
When first responders arrive, they know exactly where to look.

That’s it. Ten minutes. Zero cost. Potentially life-changing.
I think of it as a love letter to the people who might have to help you someday. And a gift to your family, who won’t have to scramble for information in the worst moment.
Print one for yourself. Print one for your parents. Drop one off for a neighbor who lives alone.
Some civic action is very, very small. This is one of them.
